APPLICATION OF NEW WATER CONNECTION

Monday to Friday

8:00am to 5:00pm (No Noon Break)

Duration

Two (2) hours and 36 minutes (excluding installation of new connection)


A. BASIC REQUIREMENTS:

  1. Duly accomplished written Application Form for new or additional connection service, with sketch of the location of the premises;
  2. Photocopy of any government-issued identification cards bearing the photo & signature of the applicant and/or authorized representative;
  3. DWD Statement of Account or DWD Official Receipt of the nearest neighbor (for routing purposes) when available;

B.    PROOF OF OWNERSHIP REQUIREMENTS:

  1. For Private Property (any of the following)
  • Land Title
  • Tax Declaration from Municipal Assessor
  • Photocopy of Notarized Deed of Absolute Sale
  • Photocopy of Notarized Conditional Sale
  • Photocopy of Notarized Deed of Donation
  • Photocopy of Notarized Contract to Sell
  • Photocopy of Building Permit specifically describing therein the location of the lot
  • Original copy of NHA Certification and City Housing
  • Original copy of Certification from Board of Liquidators
  • In case the applicant is other than the registered owner, a Special Power of Attorney (SPA) or a notarized Letter of Authority (LOA) from the registered owner shall be submitted.

      2.    For Heir in a Privately Owned Lot

  • Photocopy of Extra-Judicial Settlement of Estate
  • Photocopy of Land Title or Tax Declaration from Municipal Assessor
  • Photocopy of Notarized Affidavit of Heirship
  • Photocopy of Proof of Heirship (Birth Certificate, etc)

      3.     For Renter / Lessee of a Privately Owned Lot

  • Photocopy of current and notarized Lease Contract
  •  Photocopy of Land Title or Tax Declaration from Municipal Assessor
  • Notarized Authorization / Consent from the Lot Owner / Lessor or Special Power of Attorney
  • Notarized Affidavit of Undertaking

      4.    For Informal Settler on a Private Titled Land with Accredited Association

  • Original copy of Certification from the Association signed by the Association President (sealed)
  • Notarized Affidavit of Undertaking


       5.    For Informal Settler in the Government Property

  • Original copy of the Barangay Certification signed by the Barangay Captain with Seal, or
  • Original copy of Certification from Board of Liquidators, if applicable
  • Notarized Affidavit of Undertaking

       6.    For Corporate / Business Establishments (any of the following)

  • Lot Title (if owned) or Tax Declaration from Municipal Assessor
  • Photocopy of current and notarized Lease Contract

       7.    For Owner of a Foreclosed Property

  • Original Copy of the Certification from the Bank, SSS, GSIS or PAG-IBIG, or other housing institutions
